If your insurance provider is not listed on this page, payment is due at the time the services are rendered, and you will be responsible for filing with your insurance company. It is possible we may be providers for other carriers besides the ones listed above. If you believe this is the case we will be happy to confirm this before examination. We are out of network with, Davis Vision, Spectera, and all HMO policies.

Since our office provides routine vision care and medical health care your benefits may be different accordingly. We suggest you contact your carrier to have a better understanding of your benefits before you come in.
